  • 1. Ask the doctor. Every article you write on preventing heart disease or stroke mentions the importance of exercise. But I can't exercise due to severe lower back problems. I religiously take medications for my high blood pressure, high cholesterol, and low thyroid. There must be other people like me who can't exercise but really want to prevent heart trouble. Can you give us any suggestions?

  • 2. Ask the doctor. My doctor and I are confused by my cholesterol levels. My good cholesterol is extraordinarily high, at 94, but my bad cholesterol is on the high side, too, at 154. To resolve this good news/bad news story, my doctor ordered a C-reactive protein test which, of course, came back right in the middle of the normal range. I don't smoke or have other factors for heart disease. Would you advise me at start a statin or some other drug?

  • 7. Ask the doctor. My total cholesterol level is well below 200. My LDL is low, my HDL high, and my triglycerides and homocysteine are both in the "desirable" range. The one thing out of whack is that my lipoprotein(a) level is more than double the "normal" limit. My cardiologist wants me to take niacin to bring it down. I can't find any information on how serious a problem this is, especially in the context of otherwise good cholesterol levels and a healthy lifestyle. (I exercise 5 days a week; eat a low-fat, high-fiber diet; and am not overweight.).

Because my resting heart rate is under 50, I would need to more than double my heart rate to reach a target of 128. Does the standard formula apply to people like me?

  • 12. Ask the doctor. I have atherosclerosis in the arteries of my heart and neck, and my doctors want me to take aspirin. I also have arthritis and take Naprosyn for it every day.   • 13. Ask the doctor. A heart attack caused me to develop heart rhythm problems. These made it necessary for me to get an implantable cardioventer/defibrillator (ICD) that can shock my heart back to a normal rhythm. My medical story is a lot like Vice President Dick Cheney's, but my social situation isn't--I don't have anyone to drive me around. My doctor doesn't want me to drive. But depending on friends and family is a major problem, and we don't live near public transportation. Any suggestions?

  • 14. Ask the doctor. My blood pressure is 180/80 mm Hg. My doctor tells me this a common problem for people in their 70s, like me. The problem is that whenever I try medicines at doses high enough to get my top number under 140 mm Hg (which I understand to be the goal), I get exhausted or have other side effects.   • 19. By the way, doctor. At my last checkup, my doctor told me my blood pressure was elevated; it was 160/90. Over six weeks, I was able to get the diastolic pressure down to the low 80s through diet and exercise. But I have trouble keeping my systolic pressure down.
