  • Title: A retrospective study of IUD usage in a Malaysian family planning clinic.

    Abstract:
    This study of 914 women, 20-40 years of age, who came to the National Family Planning Board clinics in and around Kuala Lumpur for IUD's examines, 1) the characteristics of the IUD users, at 1st insertion, 2) complications associated with IUD usage, e.g. missing thread, expulsion, and pregnancy, 3) discontinuation and continuation rates. The study was done over a 2-year period. 35% of the women were between the ages of 26-30; 26.9% were 31-35 years old. At 1st insertion the parity of most women was 3-4. The largest ethnic group in the population was the Malays (38.7%), Chinese comprised 31% and Indians 26.3%. The majority, 51.6%, used the Lippes Loop, 36.8% used the Copper 7 and 5.4% used the Copper T and 1.5% used the Multiload. 27.5% used the device for less than a year and 16.2% used the device for 1-2 years. No data was available on the duration of use for 35.4% of the subjects. All the women who had the IUD inserted were married; 53.4% for more than 7 years at their 1st insertion. Incidence of missing thread was highest in those between 31-15 years old (42.9%). Missing thread was approximately 50% in those women with 3-4 children and 32.1% in those with 1-2 children. Users of the Copper IUD also experienced 3.6% incidence of missing thread as compared with Lippes Loop users who experienced only 2.7% complication. Of those who had been on the oral contraceptive pill, 42.9% experienced missing thread. Expulsion occurred in 75 subjects and was highest among women with 1-2 children and for women who had used the IUD for less than a year. Lippes Loop C and Copper 7 types of IUD were expelled more frequently. Pregnancy occurred in 55 women (6.0%). Incidence was higest in women 26-30 years of age and for women the IUD for less than 1 year. 193 women discontinued use of the IUD due to pain, discomfort, pregnancy or to have more children. Some subjects obtained other forms of contraception.
