Abstract: We report a rare case of late spontaneous extension of a posterior capsule tear from lateral traction of an endocapsular ring, resulting in dislocation of the capsular tension ring and acrylic intraocular lens (IOL) into the vitreous cavity. A 3-port pars plana vitrectomy was performed to explant the prostheses; the eye was made pseudophakic by placement of a scleral-fixated posterior chamber IOL.
