Abstract: A modification of the "in situ" saphenous vein graft above the knee is used to perform vascular access for maintenance haemodialysis when the upper arms are no more suitable. This technique is easy to perform with only one vascular anastomosis and the above knee saphenous vein is entirely available for venipuncture.
