Abstract: The authors report a 49-year-old man who presented with obstructive hydrocephalus caused by a thrombosed posterior cerebral artery aneurysm. He underwent an endoscopic third ventriculostomy for the obstructive hydrocephalus followed by endovascular occlusion for the aneurysm.
