    Abstract:
    A 48-year-old female hemodialysis patient with gram-positive mitral endocarditis developed progressive hemodynamic collapse. Echocardiography revealed a mitral annular abscess, which expanded rapidly due to connection with and pressure from the left ventricle. The mitral annular abscess had enlarged sufficiently to obstruct the mitral orifice, resulting in acute cardiogenic shock. The patient underwent and survived emergency surgical repair, and remained well on follow-up. Une femme de 48 ans sous hémodialyse souffrant d’endocardite mitrale Gram positif a développé un collapsus hémodynamique évolutif. L’échocardiographie a révélé un abcès de l’anneau mitral qui a pris une expansion rapide en raison d’un lien et d’une pression avec le ventricule gauche. L’abcès de l’anneau mitral a grossi assez pour obstruer l’orifice mitral et provoquer un choc cardiogène aigu. Le patient a subi une réparation chirurgicale d’urgence, y a survécu et demeure bien au suivi.
