    Abstract:
    BACKGROUND: A common dissatisfaction after rhytidoplasty are the remaining lateral eyelid wrinkles also known as crow's feet. METHODS: This article presents an analysis of the enlarged myectomy of the orbicularis oculi muscle used for the definitive treatment of crow's feet in 105 patients during face lifting. Myectomy involved all the lateral portion of the orbicularis oculi muscle exposed after the undermining of the face, corresponding to about one third of the whole orbital extension of the muscle. To avoid depression in the area of the excision, a free graft from the superficial musculoaponeurotic system was performed with the blepharoplasty. RESULTS: The reduction of the crow's feet was significant during the subsequent 5 years of follow-up. CONCLUSION: The enlarged myectomy allowed the treatment of a larger area in the wrinkled region without increasing the complications and with excellent results.
