    Abstract:
    In the long term therapy of mental disorders the compliance and adherence with the treatment have a great importance. Regarding the clinical studies the adherence is quite problematic, due to the fact that at least half of the patients do not fully comply with the therapy. This review focuses on the concept, the frequency and the importance of adherence and the background factors which can play role in the nonadherence. Inside of the above mentioned, it must be emphasized that the doctor-patient relationship, the characteristics of mental illness and the drug therapy, and the health belief model are the most important factors, which have special impact on the adherence with the treatment. The author summarizes the comprehensive possibilities of recognition and improvement of adherence, including the psychotherapeutic, psychoeducational and pharmacotherapeutic approaches which serve as a basis to the effective treatment. As the nonadherence to the prescribed treatment weakens the chances of recovery form the mental disorders and increases the risk of recurrence, it is very important to recognize and ameliorate the problems of adherence in the everyday clinical practice.
