Abstract: There is a potential to reduce medication errors by supporting the use of medication reconciliation through electronic Personal Health Records. The lead user method was used to identify specific goals and activities that are required to have in an electronic medication reconciliation tool and to provide a synthesized visual design that would reflect these requirements.
