Abstract: By using the fixateur externe in the fixation of bone fractures in the presence of heavily damaged soft tissue, we now have to tackle the challenge of adequate soft tissue coverage besides osteosynthesis of bone fracture. Free microvascular tissue transfer now increasingly replaces the previous method of management in which the soft tissue was reconstructed step by step. This technique is undoubtedly the solution for a lot of earlier problems, but now other questions have come up, particularly regarding time for tissue transfer.
