  • Title: Hysteroscopy necessary before IUD?

    Abstract:
    The utility of hysteroscopy prior to IUD insertion was assessed in 351 women 23-48 years of age with a normal gynecologic history. The hysteroscopy was performed during the 1st phase of the menstrual cycle, under carbon dioxide insufflation, with a panoramic telescope. Despite the fact that all the study subjects had a history of no gynecologic problems, a pathological uterine cavity was revealed in 36 (10%) women. The pathological findings included endometrial polyps in 10 women, synechia in 9 women, submucosa myoma in 8 women, and nonspecific chronic endometritis in 4 women. IUD insertion was delayed in 22 of these 36 cases with abnormal hysteroscopic findings so that treatment could be pursued. Overall, there was a significant decrease of adverse IUD effects such as expulsion, spotting, menstrual problems, and infection in the women in this study who received preventive hysteroscopy compared to women who are not screened through this procedure. It is hypothesized that major cause of the side effects associated with IUD use. If so, hysteroscopy prior to IUD insertion should be performed systematically to lower the incidence of adverse effects.
