    Abstract:
    Despite general acknowledgement of the importance of chronic prostatitis, chronic urethritis and related syndromes, we have limited understanding of the etiology of these conditions. Clinical observations suggest that infectious agents may be important, but pathogens are often difficult to identify. Thus, treatment is empirical and often ineffective. This article reviews the literature and our recent studies suggesting that genitourinary tract infections are indeed responsible for significant morbidity in men with idiopathic lower tract syndromes. The critical implication is that accurate identification of the causes should lead to specific therapy and reduce unnecessary procedures.
