Abstract: In the light of two case reports we summarize the clinical presentation, classification, diagnosis and therapy of hymenoptera sting allergy. Clinically, large local reactions are distinguished from systemic allergic reactions. The latter are divided into four degrees of severity as defined by H.L. Mueller. Diagnosis is based primarily on the history. Detection of venom specific IgE-antibodies and/or positive venom skin tests confirm the diagnosis and are helpful for identification of the responsible insect. All patients should be equipped with an emergency kit (antihistamine, corticosteroid). In the presence of a history of systemic allergic reactions, adrenaline must be added. For highly exposed patients and those with a history of threatening reactions, specific immunotherapy is recommended.
